The purpose of this letter is to make a formal complaint about the failure to provide my disabled child with a free appropriate public education. We request that you investigate this complaint and take appropriate action.

Use dates and times and mention people by name.] My child has not been recieving adequate supervision and in-class support as outlined by his Individualized Education Plan signed in January 2004. I have tried to resolve our complaints informally. We have met with the director of special education, and the superintendent. We are enclosing copies of letters and documents concerning this complaint and our attempts to solve the problem. From the letters you can get the names and addresses of the responsible school officials. Please inform us in writing of the individual who will conduct the investigation and the date the investigation will begin.
